Rib Cartilage Rhinoplasty (Autologous Costal Cartilage)
Rhinoplasty aims to shape the nose and soften the face. There are several techniques, and the surgeon chooses the ones that suit each case, such as rhinoplasty with ear cartilage, with acellular dermal matrix or with a silicone implant.
Open rhinoplasty with rib cartilage (autologous costal cartilage) can last a long time without further surgery (unless you want to change the shape later), depending on each person. It suits many nose shapes and can also be used to rebuild the nose structure.
Why Choose Rib Cartilage Rhinoplasty

- lowers the risk of infection, because it is your own tissue and fits closely with the nose structure
- causes less friction, which lowers the chance of implant extrusion
- lowers the risk of a silicone allergy
Who Is Open Rhinoplasty With Rib Cartilage Suitable For?
- people with thin nasal tissue or little nasal tissue
- people with a short nose who want more projection, where ear cartilage is not enough and rib cartilage is also needed
- people who have had several nose revisions, have had a nose infection or are at risk of implant extrusion
- people who are allergic to silicone
- people who do not want a foreign material in their nose
Is Rib Cartilage Rhinoplasty Dangerous, or Does It Affect the Body?
Rib cartilage surgery needs a general anaesthetic, given by an anaesthetist. The surgeon then makes an incision about 3cm long under the breast fold (if there is more fat, the incision may need to be longer) and takes about 1 to 2 pieces of rib cartilage from the 6th, 7th and 8th ribs on your right side, each 2cm long.
If you have had breast augmentation, tell your surgeon
so the surgery can be planned away from your breast implants
Before rhinoplasty, a chest CT scan is recommended so the surgeon can assess whether the cartilage is healthy and suitable for use in rhinoplasty.
Other checks are also needed, such as for possible complications, and to decide whether you can have a general anaesthetic.
Health Checks Before Rib Cartilage Rhinoplasty
- CT-Scan
- physical examination
- blood test
- chest X-ray
- blood clotting test (coagulogram)
- electrocardiogram (ECG)
- if this is not convenient for you, S45 Clinic offers health checks before rhinoplasty, so ask our team for details
‘Rib cartilage rhinoplasty should only be done by a surgeon with direct experience in this technique.’
Surgery by an inexperienced surgeon can raise the risk of a punctured or torn lung
This technique therefore needs careful consideration and a detailed understanding of the steps. Rib cartilage rhinoplasty also needs incisions in 2 places, on the nose and on the chest under the breast fold, so you need to take extra care of yourself. After surgery, follow the surgeon’s advice closely to lower the risk of complications.
Does the Surgeon Use All the Rib Cartilage Removed, or Is Any Left Unused and Thrown Away?
In most cases, the surgeons at S45 Clinic remove a suitable amount of rib cartilage and use all of it, leaving none, to make the most of the incision.
