This uncertainty makes the decision harder and can leave you worried about paying for more than you need.

Key points

Nasal osteotomy is a surgical technique that uses specialised instruments to move in or cut the bones at the base of the nose so that they become narrower or smoother. It is used together with rhinoplasty to help the nose look slimmer and suit the face better.

3 main types: lateral osteotomy, which narrows a wide nasal base / dorsal hump reduction, which removes a bump on the bridge of the nose / combined osteotomy, which does both in cases that have both problems.

It suits people with a large or wide nasal base, a dorsal hump on the bridge, a crooked nose from genetics or an injury, and people having rhinoplasty whose existing base does not suit a silicone implant.

Nasal osteotomy vs rasping differ in the instruments used and the purpose. Osteotomy moves the bones in to make the base narrower, while rasping (filing down bone) removes a bump on the bone. Both can be done at the same time in cases that have both problems.

The advantages are long-term results, a noticeable change to the nose shape and a lower chance of the silicone implant edges showing. The disadvantages are a longer recovery than standard rhinoplasty, more swelling and bruising, and wearing a nasal splint for 7 to 14 days.

The cost is about THB 30,000 to 80,000, depending on the case and the clinic. In most cases, this is an extra cost on top of the main rhinoplasty.

This article explains nasal osteotomy, from what it is, the 3 types of technique, how it compares with rasping, the steps, pros and cons, side effects and cost, to aftercare. It will help you judge whether you need nasal osteotomy and go into your consultation with a surgeon feeling more confident.

.kb-table-of-content-nav.kb-table-of-content-id6161_f719ae-45 Osteotomy

Nasal osteotomy, sometimes called nose osteotomy, is a rhinoplasty technique that uses specialised instruments to move in, cut or break the bones at the base of the nose. It narrows a wide bony base or reduces a bump on the bridge, so that the nose looks slimmer and suits the shape of the face better.

The surgeon uses small instruments through an incision inside the nostril or on the skin at the side of the nose, then uses an osteotome (a surgical chisel), a saw or a Piezo instrument to cut the bone along a planned line. The bones are then set into shape and the base is moved in to make it narrower. Bone that is removed does not grow back, so the new shape lasts for the long term.

In Thailand, nasal osteotomy is often done together with silicone implant rhinoplasty, because a slim, balanced nasal base helps the implant sit closely against the bone and lowers the chance of the implant edges showing or the implant becoming tilted over time. The answer is that some problems cannot be corrected with a silicone implant alone. The bone structure underneath needs to be set right first.

The nasal base is the bone at the sides of the nose that sets the width and the shape of the whole structure. If the existing base is wide, crooked or has a dorsal hump, placing a silicone implant on top cannot correct these problems. The result may look out of proportion or the implant edges may show.

Nasal osteotomy is therefore a way of “reshaping the underlying structure” so that it is ready before augmentation. The main problems nasal osteotomy can help correct are:

1. A Wide or Large Nasal Base

People whose nasal bones are wide on both sides have a nose that looks large and flat from the front. However high the silicone implant is, the nose still looks large. Osteotomy moves the bones in to make the base narrower, so the nose looks slimmer and more defined.

2. A Nose With a Dorsal Hump (Hump Nose)

A dorsal hump is bone that sticks out on the bridge of the nose, which shows as a bump from the side. Placing a silicone implant over a dorsal hump makes the bridge look larger and uneven. Removing the dorsal hump first and then augmenting gives a smooth, sloping bridge.

3. A Tilted or Crooked Nose

A nose that is crooked from genetics or an injury often has a bony base that is tilted too. With a silicone implant alone, the tilt still shows. Osteotomy helps set the bones straight and symmetrical before augmentation.

4. Preventing the Silicone Implant Edges From Showing

When the nasal base is wide, a silicone implant can look like a rod with clearly visible edges. Moving the base in to make it narrower lets the implant sit smoothly against the bone, so it blends with the shape of the nose and has a lower chance of the edges showing over time.

Who Nasal Osteotomy Suits

Nasal osteotomy is not a technique everyone needs. It is an option for cases with specific problems that a silicone implant alone cannot correct. Assessing yourself correctly helps you know whether this extra step is needed.

The surgeon assesses the anatomy of your nasal base, including its width, its symmetry and whether there is a dorsal hump, before advising whether osteotomy is needed. This assessment needs an in-person examination by a surgeon and cannot be done from photos alone.

However, you can do a first check yourself before your consultation. Signs that nasal osteotomy may be worth considering include:

A Wide Nasal Base

Use your fingers to feel the sides of the bridge near the inner corners of your eyes. If the bone feels hard and very wide, reaching close to the inner corners of your eyes, and your nose looks like a large, flat rod from the front, you may be a case where osteotomy can help.

A Dorsal Hump on the Bridge

From the side, you can see a bump in the middle of the bridge, and when you run a finger along the bridge it feels uneven, with one point higher than the rest. In this case, the dorsal hump is usually removed before augmentation to give a smooth bridge.

A Tilted or Crooked Nose

The nose looks asymmetrical, the base tilts to one side or you have had an injury to the nose before. Osteotomy helps set the bones straight and helps the result of augmentation look balanced.

Previous Augmentation With Visible Silicone Implant Edges

If you have had rhinoplasty before and the silicone implant edges show clearly from the front or the side, this may be because a wide nasal base does not suit the implant. Correcting it with osteotomy and a new silicone implant can help it blend in.

Cases That Do Not Need Osteotomy

If your nasal base is not wide, you do not have a dorsal hump and your nose is straight and symmetrical, augmentation with a silicone implant or cartilage alone is enough. It is divided into several techniques depending on the area that needs correcting. The surgeon chooses the technique that suits the features of each nose. Some cases need one type and some need several types together.

Understanding each technique helps you talk to the surgeon clearly and understand why they recommend a technique for your case. Choosing a suitable technique is an important factor in how closely the result matches what you expect.

The 3 main types used today are:

1. Lateral Osteotomy (Narrowing the Base)

This technique is used for a wide nasal base. The surgeon makes a small 2mm to 3mm incision on the skin at the side of the nose or inside the nostril, then uses small instruments to cut the bone at the side of the nose to create a fracture line. The bones are then moved in towards each other to make the base narrower. The aim is a slimmer bridge that looks more defined from the front.

2. Dorsal Hump Reduction

This technique is used when there is a dorsal hump on the bridge of the nose. The surgeon uses an osteotome or a Piezo instrument to remove the excess bone and smooths the bony base. It is often done through open rhinoplasty, so that the surgeon can see the structure clearly. The aim is a smooth, sloping bridge without the bump. For more about open rhinoplasty, see open vs closed rhinoplasty

3. Combined Osteotomy (Osteotomy and Dorsal Hump Removal)

When there is both a wide base and a dorsal hump, both techniques are done together in one operation. The surgeon removes the dorsal hump first, then narrows the base. This technique is more complex and takes longer, but it treats both problems in one operation. It suits cases that want a clear change to the shape of the nose.

Additional Technique, the Piezo Instrument

Many clinics now use a Piezo (piezoelectric) instrument for osteotomy instead of the traditional osteotome and hammer. Piezo uses high-frequency vibrations to cut bone, which helps the surgeon work precisely, reduces the effect on surrounding tissue and may support recovery. It costs more, but it suits complex cases.

Nasal Osteotomy vs Rasping, What Is the Difference

Many people confuse nasal osteotomy with rasping, because both techniques reshape the bones of the nose. Both can be done in the same operation if the case has both problems.

Before deciding which technique to have, compare them in the table below.

Comparison Table, Osteotomy vs Rasping

Feature

Nasal osteotomy

Rasping

Both together

Main purpose

Narrows the base

Removes a bony bump

Corrects both the base and the dorsal hump

Instruments

Osteotome / Piezo

Rasp / burr

Both

Suitable cases

Wide nasal base

Nose with a dorsal hump

Wide base and a dorsal hump

Result

Slimmer nose from the front

Smoother bridge from the side

Slimmer from the front and smoother from the side

Recovery

7 to 14 days

5 to 10 days

10 to 14 days

Complexity

Moderate

Moderate

High

Cost

30,000-60,000

15,000-30,000

50,000-80,000

Which Technique to Choose

If your nose looks large from the front but normal from the side, osteotomy may be the option to consider. An accurate assessment looks at the bone structure, the thickness of your skin and your goals. For more about reshaping the nose, see alar base reduction without an implant

Steps of Nasal Osteotomy, How It Is Done

Many people worry that nasal osteotomy will be frightening or very painful. In fact, it is a standard technique that experienced surgeons perform routinely. It has clear steps and uses general anaesthetic or local anaesthetic, so you are asleep or the area is numb during the operation.

The whole process takes about 2 to 3 hours, depending on how complex the case is. Osteotomy alone takes less time, while cases done with rhinoplasty or combined osteotomy take longer.

Understanding each stage helps reduce worry and helps you prepare properly. The steps are:

1. Assessment Before Surgery

The surgeon examines the bone structure of your nose, takes photos from several angles, draws up a surgical plan and explains the technique to be used. At this stage, you can ask any questions, see a simulation of the expected result and tell the surgeon about any health conditions or medicines you take.

2. Preparing Before the Day of Surgery

For 7 to 14 days before surgery, stop medicines and supplements that make you bleed more easily, such as aspirin, ibuprofen, vitamin E and fish oil, and avoid alcohol and smoking. On the day of surgery, do not eat or drink for 8 hours before the operation.

3. The Operation

The surgeon uses general anaesthetic when osteotomy is done with rhinoplasty, or local anaesthetic when osteotomy is done on its own. A small incision is made on the skin at the side of the nose or inside the nostril, specialised instruments (an osteotome or Piezo) are used to cut the bone along the planned line, and the base is then moved into shape.

4. Splinting and Closing the Incisions

After osteotomy, the surgeon places a plastic splint or another type of nasal splint over the outside of the nose to hold the bones in position, and puts gauze packing in the nostrils to stop bleeding. The incisions are closed with dissolvable stitches or stitches that need to be removed.

5. After Surgery

You rest in a recovery room for about 1 to 2 hours. Some cases under general anaesthetic may need to stay in hospital for 1 night, while cases under local anaesthetic can go home on the same day.

Pros, Cons and Side Effects

Before deciding on nasal osteotomy, weigh up the pros and cons carefully and understand the side effects you may get, so that your expectations are realistic.

Every surgical procedure has benefits and drawbacks, and nasal osteotomy is no different. A balanced assessment helps you decide whether it is worthwhile for your case.

An overview of the pros, cons and side effects:

Pros

Long-term results, because bone that is removed does not grow back / a clearer change to the nose shape than a silicone implant alone / a lower chance of the silicone implant edges showing over time / helps the face look more balanced and defined overall / corrects problems that a silicone implant alone cannot, such as a dorsal hump and a wide base

Cons

A longer recovery than standard rhinoplasty (10 to 14 days vs 7 days) / more swelling and bruising, with dark bruising under the eyes for 7 to 10 days / wearing a nasal splint for 7 to 14 days / an extra cost of THB 30,000 to 60,000 on top of rhinoplasty / higher complexity, so choose a surgeon trained in this field

Side Effects You May Get

Swelling and bruising in the first 7 to 14 days, which is normal / dark bruising under the eyes that fades within 2 weeks / temporary numbness around the nose for 1 to 3 months / minor nosebleeds in the first 2 to 3 days / a clear runny nose in the first week

Side Effects That Need a Doctor Straight Away

If you get unusually severe pain, redness, swelling and warmth, a fever, fluid or pus coming out, or bleeding that does not stop for more than 3 to 4 days, see a doctor straight away, because these may be signs of infection or a complication.

Is Nasal Osteotomy Risky

Overall, this technique can be done appropriately when it is carried out by an experienced surgeon trained in this field at a clinic that meets standards. It depends on the bone structure of each case. If your nasal base is not wide, you do not have a dorsal hump and your nose is straight and symmetrical, a silicone implant alone is enough. But if you have a wide base, a dorsal hump or a crooked nose, osteotomy can help the result look more balanced. An experienced surgeon will assess you and advise based on your actual case.

Does Nasal Osteotomy Hurt?

During the operation, you are asleep under general anaesthetic or the area is numbed with local anaesthetic. After surgery, you may get a dull ache for the first 2 to 3 days, which can be managed with painkillers prescribed by the surgeon. The ache gradually eases during the first week. Most people find the nasal splint and temporary nasal congestion more uncomfortable than the pain.

Can Nasal Osteotomy Be Done With Rhinoplasty?

Yes, and it is a common approach. In most cases, osteotomy is done together with rhinoplasty in one operation, because the time right after osteotomy is a suitable moment to place the silicone implant closely against the new bone structure. Having both in one operation saves recovery time and cost compared with having them separately.

Can Nasal Osteotomy Be Done Without a Silicone Implant?

Yes, in cases that only need the base narrowed without adding height, such as people who already have a high bridge but a wide base. However, this is uncommon. Most people who decide on osteotomy want to change the overall shape of the nose, so it is done together with augmentation in one operation.

What Are the Disadvantages of Nasal Osteotomy?

The main disadvantages are a longer recovery than standard rhinoplasty, wearing a nasal splint for 7 to 14 days, more swelling, bruising and dark marks under the eyes in the first 1 to 2 weeks, an extra cost of THB 30,000 to 60,000 on top of the main rhinoplasty, and higher complexity, which means choosing an experienced surgeon is important.

How Long Does It Take to Recover From Nasal Osteotomy?

Most swelling and bruising goes down within 7 to 14 days, and you can go back to work after the splint is removed. The shape gets close to its final shape in months 1 to 3, and the full result is usually clear in months 3 to 6 after surgery, when the bones that were cut have fully stabilised. Timings depend on each person.

How Much Does Nasal Osteotomy Cost?

The cost is about THB 30,000 to 80,000, depending on the technique and complexity. Osteotomy to narrow the base alone is THB 30,000 to 60,000, dorsal hump reduction is THB 25,000 to 50,000 and combined osteotomy is THB 50,000 to 80,000. These prices are an extra cost on top of the main rhinoplasty. Ask the clinic for a quote that lists everything clearly.

Nasal Osteotomy vs Filler Injections, What Is the Difference?

They are completely different. Nasal osteotomy reshapes the bone structure to make it narrower and gives long-term results. Filler injections put a filler substance under the skin to add height in specific areas, and the effect is temporary, lasting about 8 to 18 months. Filler injections cannot reshape the bone structure or narrow a wide nasal base. If you want a significant change to the shape of your nose, consider osteotomy or surgical augmentation.

Summary

Nasal osteotomy is a surgical technique that moves in or cuts the bones at the base of the nose to make it narrower or smoother. There are 3 main types: lateral osteotomy to narrow the base, dorsal hump reduction, and combined osteotomy, which does both at the same time. It suits people with a wide nasal base, a dorsal hump on the bridge or a crooked nose.

This technique differs from rasping, which focuses on filing down bone that sticks out, while osteotomy focuses on moving the bones in to make the base narrower. Both can be done at the same time in cases that have both problems. The cost is about THB 30,000 to 80,000, depending on the case and the clinic. In most cases, it is done together with rhinoplasty in one operation to save time and cost.

The most important thing is to choose an experienced surgeon trained in this field and a clinic that meets standards, because osteotomy needs precision and a small error can affect the shape and symmetry of the nose in the long term. A consultation with a surgeon before you decide will help you find out whether you need osteotomy and plan suitable treatment.

Because every nose structure…
is different

An otolaryngologist with subspecialty training in facial plastic and reconstructive surgery assesses every case before treatment and gives advice based on your actual concerns.

“Reveal the beauty within you”

Chat with us on LINE

Message us online

Call us +66 2 125 2543

S45 Clinic (Sukhumvit 33)Open daily 8:30am to 11:30pm.
